TT Fuse holder

Can anyone please either take a pic or describe the fuse holder. Mine has been in the pile of poop since day one. I had taped it to keep it tight but that has finally failed. I took it out and there was one bent prong. I thought it had more. Weird -- at anyrate she is dead til I get a replacement fuse holder.
