Intermittant problems can be a bitch. It sounds like either a HV connection or a resistor. It needs a good going over by a tech. Probably why it was traded in anyway...

Probably a bad preamp tube. It could also be the input jack or its connections. Several other possibilities actually but those are most common. Try pulling the first tube (closest to the input jack) and reseating it. If that fixes it, there you go. Otherwise, try replacing it for a fresh 12AX7.

It can be confusing about fixed bias, adjustable bias and cathode bias, mainly because fixed sounds like it should be the opposite to adjustable, but it isn't. I think it is easy to get more than 100W out of 4 EL34s in Class AB by using high plate voltages and having each output valve dissipate greater than 25W for only part of the cycle. If mount a lpad like normal would this be ok as then from what people have said I can keep on the 4 setting and use the lpad to attenuate instead of the built in one? I understand what you are saying, but that's not why it is happening. The other speaker sounded different because it was different.
